Introduction: Bad breath refers to unpleasant breath odors originating from the mouth. Because it plays an important role in social communication, managing bad breath is important from both social and health perspectives. Halitosis is a common human problem and may cause social and psychological difficulties for affected individuals. It is also positively associated with the accumulation of bacterial plaque in the oral cavity. Objective: This study aimed to evaluate the effect of tongue brushing on halitosis among adolescents aged 15-20 years in high schools in Sana'a city. Methods: This cross-sectional, closed-ended questionnaire-based study was conducted in Sana'a city from January 12, 2025, to January 24, 2025. The study included 113 adolescents with a mean age of 17 +/- 3 years. Data were collected using an online questionnaire completed by the participants. Results: The results confirmed the importance of regular tongue cleaning, frequency of tongue cleaning, use of toothpaste, regular toothbrushing, frequency of toothbrushing, duration of toothbrush use, and use of a specific tongue brush. These variables showed a strong relationship with bad breath. Conclusion: The study indicated that participants were generally aware that failure to brush the tongue may increase bad breath and related complaints, although some uncertainty remained. Therefore, awareness should be raised about oral changes associated with inadequate tongue cleaning and the importance of tongue brushing in reducing or eliminating bad breath. The findings also emphasize the need to promote tongue cleaning and oral hygiene practices among adolescents.
